Please note, if you are in danger do not use this form but immediately report the emergency to local law enforcement.

Welcome to the neighborhood! If you are lucky enough to be a THPRD resident, you are able to take advantage of the hundreds of classes, activities, sports leagues, parks and trails throughout the largest special park district in Oregon.
All you need to register for any of the classes, teams or activities is a THPRD Residency Card. To apply for the card, simply print out and complete the Registration Card Information Form. Next, bring it to your local THPRD facility with the two forms of verification listed below.
- Driver's license or DMV identification card
- Proof of residential address, such as utility bill, lease agreement, credit card statement, insurance card or property tax statement
You must complete this process in person at a facility.
Don't delay. Allow about two weeks from the date of application for processing. If you are a resident, the card is free. Once you have your card, you may register for classes online, by phone, by mail or fax.
